Our Care Managers provide routine remote appointments in compliance with Medicare guidelines for patients with two or more chronic conditions. They determine if a patient qualifies for the program, complete onboarding, initiate customized care planning, and engage with the patient to foster a working relationship toward their optimal health. Billing is submitted by our business team, relieving the practice of that administrative burden. Providers receive regular progress reports, allowing for timely intervention when a patient’s condition changes. Practices gain a new revenue stream through billable CCM services.
